Back Pain - what is the role of Weight in its development?
As chiropractors, we frequently encounter patients struggling with back pain, a prevalent condition that affects millions worldwide. While the causes of back pain can vary, one significant factor that often contributes to its development is weight.
Weight plays a crucial role in spinal health and overall well-being. At the outset it is important to note that being heavier than recommended does not automatically mean you have back pain. Plenty of people of all weight ranges can have or not have back pain. However, excess weight places additional stress on the spine, joints, and muscles, leading to increased wear and tear over time. This additional strain can result in various spinal issues, including:

- 1. Back Pain from Increased Pressure on the Spine: Excess weight can cause the spine to compress, leading to discomfort, stiffness, and pain. The lumbar region, in particular, bears the brunt of this pressure, as it supports the body's weight and absorbs impact during movement.
- 2. Risk of Disc Degeneration: The spinal discs act as cushions between the vertebrae, absorbing shock and facilitating movement. However, excess weight can accelerate disc degeneration, causing them to lose height and maybe bulge. These discs can then create problems such as nerve pressure or shear and move in unsupported ways that cause arthritic change.
- 3. Back Pain from Muscle Imbalances: Carrying excess weight can lead to muscle imbalances, where certain muscles become overworked while others weaken. This imbalance can affect posture and spinal alignment, contributing to back pain and discomfort.
- 4. Increased Risk of Chronic Conditions: Obesity is closely linked to several chronic conditions, such as osteoarthritis, diabetes, and cardiovascular disease. High weight is linked with systemic inflammation which contributes to the development of metabolic dysfunction and disease ( Obesity and Metabolic Syndrome 2012). These conditions can exacerbate back pain and hinder the body's ability to heal and recover.
- 5. Back Pain from Postural Change: Weight gain is not deposited proportionately over the body. A large percentage of the gain is placed on the abdomen creating a forward shift in the body's centre of gravity. The body adjusts for this by leaning a little backwards at the lower back. This does a few things. Firstly, more load gets placed on the vertebral facet joints which are more designed as guides than weight bearers and so there can be an acceleration of arthritic change. Secondly, the mid back and neck curves have to alter to adjust for the greater curve below them. This creates new unaccustomed loads that the structures were not well designed for.
Addressing weight-related back pain requires a multifaceted approach that focuses on lifestyle modifications, exercise, and proper spinal care. Here are some strategies to consider:
- 1. Maintain a Healthy Weight: Adopting a balanced diet and engaging in regular exercise are essential for weight management and overall health. Incorporate strength training, cardiovascular exercise, and flexibility exercises into your routine to support spinal health and reduce back pain.
- 2. Practice Good Posture: Be mindful of your posture throughout the day, whether sitting, standing, or lifting objects. Maintain a neutral spine position, keep shoulders relaxed, and avoid slouching or hunching forward, which can strain the back muscles and ligaments.
- 3. Invest in Ergonomic Support: Use supportive ergonomic furniture and tools, such as ergonomic chairs, standing desks, and supportive pillows, to promote proper spinal alignment and reduce strain on the back.
- 4. Seek Chiropractic Care: Regular chiropractic adjustments can help alleviate back pain, improve spinal alignment, and enhance mobility. Chiropractors use gentle, non-invasive techniques to address spinal misalignments, reduce inflammation, and promote healing.
- 5. Stay Active: Incorporate low-impact activities like walking, swimming, or yoga into your routine to condition muscles, improve flexibility and support overall spinal health.
